The screening process was iterative and thus excluded individuals if they belonged to a demographic stratum for which the recruitment quota had already been met. Other exclusion criteria included those under age 18 and those unable to be interviewed in English and by telephone. Of these contacts, screening interviews were completed, individuals agreed to participate, gave verbal informed consent and were then mailed a survey booklet.
